Beyond economic inequality: What about *structural* inequality?

I stumbled across this Reuteres article today from about a year and a half ago (February 2014) and I feel that it speaks to something that has always bothered me about the "economic inequality is the most important issue in America, bar none" but I couldn’t quite put my finger on….
